New Statement Screen
Use this screen to create a statement for the selected workfile. A statement includes the person contact information and relevant information regarding the loss. The contact information must first be entered in the Contacts tab of the workfile.
Description of fields on the screen
| Field Label | Description |

| Statement From | Use the droplist to select the party. The person information must first be entered in the Contacts tab before you can make a selection here. |
| Statement Type | Select the type from this droplist. Valid values include Verbal / Non-recorded, Verbal - Recorded, Video, and Written. |
| Date | Enter the date or select it from the calendar icon. Also, select the time from the droplist. |
| Title/First Last | The title option is a droplist. Enter the person name in the fields. |
| Company | Enter the name of the company if it is relevant. |
| Phone Number(s) | Enter one or more phone number. Use the blue arrows to arrange the order according to preference. Selecting the red X button removes the selected phone number from the list. |
| Phone Number(s) | Phone Type, Phone Number, and Order. |
| Statement | Enter the statement information in this field. |
| OK | Select this option to save the information and close the screen. |
| Cancel | Selecting this option closes the screen without saving. |
